In April 2004, United Nations Secretary General Kofi Annan appointed an independent, high-level inquiry (chaired by Paul Volcker) to investigate the administration and management of the Oil-for-Food Programme in Iraq. The decision was endorsed by the Security Council (Res. 1538 (2004)). [ source ]
